Policy numbers are in this format: ABCD-123456789.  If you purchased your policy online, look for a confirmation message in your email inbox.  If you are still having difficulty finding your policy number, you can also call or visit a nearby location.

This is the Date of Birth of the primary person on your policy.   The primary policy holder must be at least 16 years old.
